Registration: Tuesday May 4th 6:30 to 8:00PM

Everyone needs the following forms on file with the office by May 5th:

1. Member Registration Form (Youth)
    Member Registration Form (Adult)

2. Member Medical Form (Youth and Adult)

3. Camp Registration Form (Youth)
    Camp Registration Form (Adult)

4. Camp Code of Conduct Form (Word 2003 file)

5. Medication List Form (Word 2003 file) (Please fill out and bring with you to Camp.)

6. Background Check (For Adults ONLY. Must be turned in ASAP.
